Advantage Program

Blue Cross of Idaho members are entitled to special pricing of $895 per eye for conventional LASIK and $1295 per eye for custom LASIK or PRK from the Idaho network.  These fees include all pre- and post-operative care for one year after surgery.  Members do not have to submit a claim form or documentation, but must show their Blue Cross identification card at the time of service to receive this special pricing.
