Disease management is dedicated to improving members' health and quality of life.  Our disease management programs focus on members at the highest risk for developing complications from chronic disease. By encouraging self-management and facilitating optimal care with doctors, our disease management programs can help members achieve cost-effective healthcare that promotes a greater quality of life. 

The goal of disease management is to offer intervention and education. We encourage members to play an active role in managing their illness and include their doctors in the process.
