  • Your Bank Information for Automatic Monthly Bank Deduction (Optional)
  • Which plan you are applying for
Application Requirement:
  • You must be an Idaho resident
  • And you must be applying during your open enrollment period (Your Medicare supplement open enrollment period lasts for six months. It starts on the first day of the month you are both: Age 65 or older and enrolled in Medicare Part B). If you have questions, or need more information contact your local Blue Cross of Idaho District Office.
