Preferred Blue® PPO

Preferred Blue is a PPO health care plan for large groups (51+ employees). Preferred Blue offers choice and flexibility as well as lower premiums and out-of-pocket expenses.

Enrolled members can save money by using contracting health care providers (doctors, hospitals, and other covered providers.) In-network physician office visits, including visits for wellness and preventive services, are covered after a $20 - $40 copayment. Additional services provided in the physician’s office such as lab, X-ray, and other diagnostic services, are subject to deductible and coinsurance requirements.

Preferred Blue members may choose to see out-of-network providers at any time. However, out-of-network services are subject to deductibles, higher coinsurance, and higher out-of-pocket maximums. Members may also be responsible for provider charges that exceed the maximum allowance. As Preferred Blue enrollees, your employees make the choice.
