Preferred Blue^® PPO

Preferred Blue is one of our PPO plans for small groups (2-50 employees). Preferred Blue offers choice and flexibility, as well as lower premiums and out-of-pocket expenses.

Enrolled members can save money by using contracting health care providers (doctors, hospitals, and other covered providers). In-network physician office visits, including visits for wellness and preventive services, are covered after a $20 - $40 copayment. Additional services provided in the physician’s office such as lab, x-ray, and other diagnostic services, are subject to deductible and coinsurance requirements.

Preferred Blue members may choose to see noncontracting providers at any time. However, it is more expensive. Out-of-network services are subject to deductibles, higher coinsurance, and higher out-of-pocket maximums. Members may also be responsible for provider charges that exceed the maximum allowance. Also, there are some services that Preferred Blue does not cover when noncontracting providers are used.
