With Dental Blue PPO, you are free to choose any dental care provider inside or outside the Preferred Provider Organization (PPO) network, although you maximize your benefits when you use providers that have a Dental PPO contract with Blue Cross of Idaho. Fill out, sign and date the application and return it to your local district office or to Blue Cross of Idaho at, P.O. Box 7408, Boise, ID 83707-7408.
