Our PPO members can seek care from whomever they choose, but can keep out-of-pocket expenses down by seeking care from contracting providers.

Preferred Blue® PPO

Preferred Blue is one of our PPO plans for small groups (2-50 employees). Preferred Blue offers choice and flexibility, as well as lower premiums and out-of-pocket expenses.

Enrolled members can save money by using contracting healthcare providers (doctors, hospitals, and other covered providers). In-network physician office visits, including visits for wellness and preventive services, are covered after a $20 - $40 copayment. Additional services provided in the physician’s office such as lab, x-ray, and other diagnostic services, are subject to deductible and coinsurance requirements.

Preferred Blue members may choose to see noncontracting providers at any time. However, it is more expensive. Out-of-network services are subject to deductibles, higher coinsurance, and higher out-of-pocket maximums. Members may also be responsible for provider charges that exceed the maximum allowance. Also, there are some services that Preferred Blue does not cover when noncontracting providers are used.

Preferred Blue is a PPO healthcare plan for large groups (51+ employees). Preferred Blue offers choice and flexibility as well as lower premiums and out-of-pocket expenses.

Enrolled members can save money by using contracting healthcare providers (doctors, hospitals, and other covered providers.) In-network physician office visits, including visits for wellness and preventive services, are covered after a $20 - $40 copayment. HSA Bluesm PPO

The introduction of Health Savings Accounts (HSAs) has opened the door to a new breed of healthcare coverage. Paired with a high deductible health plan, an HSA works with you to manage your current and future healthcare needs. You can participate more actively in your healthcare spending decisions because the money in the account belongs to you.

HSA Blue is a Preferred Provider Organization (PPO) plan that meets the federal requirements for an HSA-compatible plan.

Financial Benefits

The beauty of an HSA is that contributions roll over each year and are portable if you should change jobs. Funds in an HSA can be used to help pay your deductible and out-of-pocket medical expenses. Any money left over will remain in your HSA account and grow tax-deferred. Withdrawals from your HSA for eligible medical expenses are exempt from federal and state taxes and contributions you make to your HSA, up to a specified limit (currently the lesser of the deductible or $3,100 a year for individuals and $6,250 a year for families for 2012), are tax-deductible at both the federal and state levels.

The choice of healthcare providers is yours. HSA Blue has both in-network and out-of-network benefits. To get the most in benefit payments, you should choose providers from our PPO contracting network. Our extensive provider network will ensure that you receive quality healthcare at affordable rates.

Prescription Drug Options

Major Medical

For plans with a $2,000 or $3,000 deductible, we pay 60% of the maximum allowance for covered prescription drugs after you have met your deductible. With a $5,000 deductible, we pay 100% of the maximum allowance for covered prescription drugs after you have met your deductible. When you go to your pharmacy, you will pay the entire cost of the drug. To be reimbursed, fill out a claim form and return it by mail.

An HSA administrator can help you set up and manage a health savings account that will provide the greatest financial benefit to you. Please note that Blue Cross of Idaho is not an HSA administrator. Rather, we have created HSA Blue to meet the requirements of a federally qualified high deductible health plan. If you need help finding an administrator, call your local Blue Cross of Idaho district office or talk to your insurance agent.

Blue Value is our most affordable managed care plan. Choose from several copayment, deductible and out-of-pocket limit options.

Blue Valuesm

Blue Value is our most cost-effective managed care plan with ten levels of coverage to choose from. Primary Care Physician (PCP) office visits to contracting providers are covered after a $20-$30 copayment while non-PCP office visits to contracting providers are covered after a $40-$50 copayment. It is important to note that with Blue Value, the office visit copayment does not cover lab, x-rays or other diagnostic services. As with other services received in-network, members are responsible for 30 or 40 percent coinsurance after they have met their deductible.

For out-of-network services, members are responsible for 50 or 60% coinsurance after meeting a separate, out-of-network deductible.

As with our other managed care plans, your employees will need to select a PCP from our statewide network of contracting providers. PCPs can be general practitioners, family practitioners, internists, pediatricians, and obstetricians and gynecologists. They oversee the healthcare of your employees and their enrolled family members while providing preventive care, immunizations and other healthcare services.

We encourage our managed care members to coordinate their healthcare through their PCP, but they do not need a referral to seek care from other contracting providers.

Blue Valuesm

Blue Value is our most cost-effective managed care plan with many levels of coverage to choose from. It is important to note that with Blue Value, the office visit copayment does not cover lab, x-rays or other diagnostic services. As with other services received in-network, members are responsible for 10, 20 or 30 percent coinsurance after they have met their deductible.

For out-of-network services, members are responsible for 30, 40 or 50 percent coinsurance after meeting a separate, out-of-network deductible.

As with our other managed care plans, your employees will need to select a PCP from our statewide network of contracting providers. PCPs can be general practitioners, family practitioners, internists, pediatricians, and obstetricians and gynecologists.
